This forum is part of the City’s Community Health Needs Assessment and Community Health Improvement Plan process — and your input matters.

With your voice, the city can:

• Help shape future health policies
• Direct the allocation of resources
• Respond effectively to public health needs
• Align programs with community priorities
• Improve overall health outcomes
